Appointments

We take pride in customizing an appointment time to fit into your life. Please let us know as soon as possible if you are unable to keep your appointment. A $75.00 fee can be charged without at least a 24 hour notice.

Payments

We accept cash, checks, Visa, Mastercard and Discover. Payments and insurance copays are due on the day of service. We offer a 5% discount for payments made in full on the day of your procedure.

Care Credit is the financing option we offer for your convenience.

Insurance

Our office accepts all dental insurance policies. We are a preferred provider for Delta Dental of Wisconsin, Blue Cross Blue Shield and Humana. Balances above and beyond insurance coverage are patients responsibility.
