Appointments

We will charge a $75.00 fee for no shows or cancellations within 24 hours of an appointment.

Payments

We are committed to the success of your treatment. Please understand that your payment of your bill is considered a part of your commitment to treatment. In order to be impartial to everyone we require payment at the time of service. This includes your copayment and deductible. We accept cash, checks, Visa, MasterCard and Discover. For extensive treatment plans we offer in office as well as Care Credit payment plans.

Insurance

We accept all dental insurance policies, however, we are only network providers for the following: Delta Dental of Wisconsin, Blue Cross Blue Shield and Humana. Account balances are your responsibility whether or not your insurance pays for treatment. It is also your responsibility to inform us of changes in your insurance coverage.

Financing

Balances after 90 days are charged a yearly finance charge of 18% (That is equal to 1.5% of your outstanding balance per month).
